The Commonwealth of Virginia requires The Williams School to have a completed Medication Consent Form to accompany each medication, either over-the-counter or prescription, which is administered to your child by the school. If you currently have medications at school, you have received a call from Annie and a letter from the school which included the necessary form. The forms are also available in the front office as well as here on the web site and include complete instructions. The Commonwealth mandates that all consents be returned by January 1 each year.
For prescription medications, the Medication Consent must be completed by the prescribing physician. If you are taking your child to the doctor for a sick visit, please take a form with you. If you are providing over the counter medication for the school to give to your child, you will need to fill out the appropriate section of the form.
